If you mean are they all stretched by gradually increasing the size of the jewelry over an extended period of time, then yes. Earlobes can also be stretched using weights in some cases, although this is not ideal for obtaining healthy lobes. The only other way to achieve a large-gauge piercing is by dermal punching or scalpelling, which is the process of turning two earlobe piercings into one larger hole by cutting between them, either in a straight line, or by removing a portion of tissue. Alternatively, making a smallscalpelcut outward from an existing earlobe piercing is one method used to enlarge a piercing very quickly, without the time needed for stretching. Or in the case of scalpelled labrets, it is an initial large gauge labret cutting.
